Geriatric Assessment Center

The Geriatric Assessment Center provides comprehensive medical assessment and diagnostic services for senior citizens who are experiencing problems with activities of daily living (ADL).

  • Gait and balance disorders
  • Behavioral changes
  • Episodes of confusion
  • Wandering
  • Safety issues
  • Incontinence
  • Multiple medications or medication management
  • Decline in self-care abilities
  • Increased caregiver dependence, institutional placement being considered
  • Frequent doctor or hospital visits

If so, The Geriatric Assessment Center may be able to help. All these issues are typical occurrences for many older adults and can lead to illness and injury, as well as increased stress for the individual and his/her caregiver(s).

Geriatric Assessment Services

  • "Welcome to Medicare" physical exam (available during the first 6 months of obtaining Medicare Part B coverage)
  • Medical screening
  • Functional screening
  • Screening for depression
  • Gait and balance assessment
  • Memory impairment assessment
  • Dementia evaluation
  • Mild cognitive impairment evaluation
  • Psychosocial evaluation
  • Medication assessment
  • Treatment plan with recommendations
  • Osteoporosis screening and treatment
  • Continuity of care with a primary care physician

Seniors should be referred by their personal physician.

Medicare and direct payment are accepted. The Geriatric Assessment Center at Griffin Hospital accepts all forms of private insurance, and the initial assessment visit is covered 100% by Medicare.
